地尔硫控制性降压在口腔颌面外科手术中的应用

【摘要】 目的 通过应用地尔硫控制性降压观察其对血流动力学的影响,评价其降压效果及安全性。方法 选择颌骨切除术患者25例,比较降压前后血液动力学变化。结果 与降压前比较,地尔硫降压时,HR、MAP、SVR变化较显著,SI、SV、CO、CI无显著性变化,降压停止后,无血压反跳现象。结论 地尔硫降压无明显不良反应,可控性好,安全有效。

【Abstract】 Objective To investigate the effect of controlled hypotension on hemodynamics by making use of diltiazem.Methods Choose twenty-five cases of department of maxillectomy, compare with the change of hemodynamics after decompression.Results After using diltiazem, HR, MAP, SVR changed significantly, and SI, SV, CO, CI changed nothing. When stop decompression, there was no blood pressure jerking.Conclusion There is no obvious side effect and it is easy to control when controlled hypotension by using diltiazem . So it is safe and effective.

口腔颌面部肿瘤手术时,由于头颈部血管丰富,出血较多,输血常难以避免。以往的经验提示:切除一侧上或下颌骨时,数十分钟出血500ml不少见[1]。通过控制性降压可减少术中出血,使手术野“干净”,有利于手术操作。同时也减少输血和降低血液传播性疾病的发生率。寻找理想的控制性降压药是保证患者手术安全的重要因素之一
